Our Story
The story of Coastal Family Advocacy and Facility Oversight (CFAFO) is a story of protection, presence, and peace of mind.
CFAFO was created from a simple but deeply important realization: families should never feel alone, confused, or powerless when someone they love enters a skilled nursing facility, assisted living community, rehabilitation center, or hospice program.
Healthcare systems can be difficult to navigate. Medical information may be overwhelming, communication may be inconsistent, and families—especially those who live far away—may struggle to understand what is happening or whether their loved one’s needs are being fully addressed. CFAFO was established to help close that gap.
Founded and led by an experienced nurse practitioner, CFAFO provides families with an independent, knowledgeable advocate who understands both the clinical and personal sides of care. Through facility visits, health and safety observations, care-plan participation, communication with healthcare teams, transition support, and clear family updates, CFAFO helps families remain informed and meaningfully involved.
The organization is not employed by the facility. Its responsibility is to the individual receiving care and the family who entrusted CFAFO with their loved one’s oversight. Every concern is approached with professionalism, compassion, careful observation, and respect for the resident’s dignity and rights..
At its heart, CFAFO was created to be the trusted presence families wish they could provide themselves—someone who can ask informed questions, recognize potential concerns, promote communication, and help ensure that the person behind the medical record is always seen, heard, and valued.
CFAFO does not replace the resident’s healthcare providers, facility staff, or family. It strengthens the connection among them.
